New Student Registration Info
School Tours
We invite you to come meet our principal, and view our beautiful campus. There will be several school tours planned throughout the year. Each tour lasts approximately 45 minutes.
The current school tour dates and times are:
- February 06, 2012 at 9:00 am
- February 13, 2012 at 9:00 am
- March 01, 2012 at 12:30 pm
- March 26, 2012 at 9:00 am
- April 12, 2012 at 9:00 am
- May 07, 2012 at 9:00 am
All forms required for registration are available in the school office. Note: Before completing any of the forms, please look over and read all information.
Please be aware that an ORIGINAL of your child's BIRTH CERTIFICATE, BAPTISMAL RECORD or PASSPORT is required to verify birth date. (The original documents will be copied at the school office and returned to the parent.) Hospital certificates are not acceptable proof of age. Incoming 2012-2013 kindergarten students must have been born ON OR BEFORE DECEMBER 2, 2007.
Items required for all new student registrations
- Registration Form (white)
- Immunization Record (blue) - complete top portion only
- Permanent Health History Card - complete both sides
- Report of Health Exam for School Entry (required for first grade)
- Birth Certificate, Baptismal Record or Passport
- Immunization Records
- Two (2) proofs of residency (escrow papers, rental/lease agreement, utility bills - note: phone bills do not prove residency)
- Student Characteristics Card
Incoming Kindergarten student immunization requirements
| Polio | 4 doses, or... | 3 doses meet requirement if at least one was given on or after the 4th birthday |
| DPT | 5 doses, or... | 4 doses meet requirement if at least one was given on or after the 4th birthday |
| MMR | 1 MMR plus 1 dose of measles | Usually both given as MMR. Both must be on or after the 1st birthday |
| Hepatitis B | 3 doses | Series must be started and up to date |
| Varicella (chicken pox) | 1 dose | Or health provider documentation of immunity |
Parents must have a copy of an OFFICIAL immunization record signed by a doctor's office or clinic. If immunizations are incomplete, bring a doctor's note stating dates scheduled. Series of immunizations must be complete or up-to-date prior to entry.
